The Advance Care Planning Coalition of Eastern Virginia (ACPCEVA) represents a unique collaborative partnership of health and human service providers with a mission to fully engage the community in the awareness, understanding, and appreciation of individual participation with Advance Care Planning. Through its As You Wish Advance Care Planning program, the Coalition promotes a unified approach among public and private health care providers and related stakeholders to encourage the public to plan ahead for end-of-life choices and the completion of advance directives for health care.
